The Uninsured Program

There are over 70,000 adults in Guilford County that have no health insurance and are unable to afford private insurance. Partnership for Health Management's Uninsured Program is here to help you improve and maintain your health. To be eligible for the program you must be 19 years or older, income eligible, live in Guilford County and can not have any form of health insurance, including Medicaid or Medicare. You must have an annual income at or below 200% FPL (federal poverty level which is for an individual $21,780 and a family of four (4) $ 44,700.

Eligibility & Enrollment Criteria:

All persons referred to Guilford Community Care Network (GCCN) must go through a brief application process to determine eligibility for the program. In order to be eligible for Project Access of Guilford County, you must provide these items at the time of your appointment:
  • Picture ID
  • Social Security Card (if applicable)
  • Proof of Income:
  • At least one of the following, but including as many as needed:
    • 4 current check stubs
    • Statement written on company letterhead, or letter including employer's name, signature and phone number
    • Award letters, including but not limited to: Social Security benefits award letter, Child support letter, Medicaid Denial letter
    • Notarized Statement from person or agency assisting with living expense
  • Current Bank Statement
  • Last years tax return (Form 1040)
  • Retirement/IRA/401K Balance statements
  • Rental Property information

 

Statistics

Guilford County covers 649 square miles and has 446,189+ residents. Of the estimated 446,189 people living in Guilford County, approximately 73,215 individuals are un/underinsured, according to the Cecil G. Shep Center for Health Services Research at UNC Chapel Hill. 88.5% of those with a household income of $50,000 or more reported having at least one health care provider, as compared to 72.4% of those with a household income of less than $50,000.




According to a report by the Guilford County Department of Public Health - The State of Guilford County's Health 2006 - The percentage of adults with health insurance has dropped from 88.6% in 2001 to 81.4% in 2005. Currently, 10% of children and 19% of adults are estimated to be without health insurance, totaling more than 11,000 children and 52,000 adults.

Partnership for Health Management Uninsured Program

Partnership for Health Management-Uninsured Program is a program that helps people without health insurance access comprehensive medical care. It is NOT a Health Insurance. The program is made up of a network of generous doctors and other healthcare providers. These doctors provide healthcare services to eligible adults.

The Uninsured Program Provides the Patient:

  • Coordination of healthcare needs
  • Coordination of specialty care needs
  • Medical Interpreters
  • Intensive and individualized case management services
  • Onsite eligibility and enrollment
  • Assistance with program approved medications, up to $750.00 a year with a $6.00 co-pay.
  • Assistance with prescriptions for eyeglasses
  • Assistance with dental care referrals
  • Assistance with a medical home
